Palliative Care

DREAM-HOPE Palliative care nurse coordination includes tertiary prevention and relieve suffering, and to support the best possible quality of life for client/patient and their families, regarding of the stage of the disease or the need for other therapies. DREAM-HOPE will support patients’ and families’ goals for the future; including their hopes for a care of life-prolonging, as well as their hopes for peace and dignity throughout the course of the illness, the dying process, and death.
