Medical Doctors (MD)

Medical Doctors (MD) diagnosis client conditions using examinations and tests, Based on their findings, they prescribe treatment and medication to client to attempt to heal an illnesses or injuries. They can specialize in number of medical areas, such as pediatrics, anesthesiology or cardiology, or other areas or they can also work as general practice physician. Becoming a medical Doctor requires earning a doctoral degree in medicine and participating in clinical rotation.

Physician

A Physician is a professional who has earned a degree of doctor of medicine or doctor of osteopathy and has passed a licensing examination. Most physicians specialize their practice of medicine. Nurses work closely with physicians under supervision or as collaborators.

Nurse Practitioner

Nurse practitioner provide health care to clients, usually in outpatient or community settings. Adult nurse Practitioners provide primary care to adults; family Nurse Practitioners provide primary care for families; Pediatric Nurse Practitioners provide care for infants and children; Gerontology Nurse Practitioners(GNP)provide care to the elderly population, and obstetrics-gynecology nurse practitioners provide primary care for women. They have Master of Science in Nursing (MSN) or Doctorial Nursing Practitioner (DNP) in nursing education.

Dentists

Dentist or Dental surgeon, is a medical care professional that is responsible for a number of preventive and restorative oral procedures. Dentist works with clients to maintain and restore health to everything within the mouth. Dentist are working in the community office, hospital dentistry, dental research centers, military and educational institution, and as in their own office. Dentist inform their clients on how to prevent dental problems, and health education on dental care and gums.

Other health care team or practitioners: Pharmacist, Physical Assistant, Therapists, social workers, case Managers, chaplain

Pharmacist

Pharmacist are licensed to formulate and dispense drugs. They have Bachelor of Science degree or doctorates in pharmacology. Pharmacist provide valuable information to nurse about drugs, and their use and effect.

Physician Assistant

Physician Assistants have medical training and work under the direction of physicians or MD in the hospitals, clinics, and private offices. DREAM-HOPE Nurse work with them as they do with Medical Doctors (MD and Physicians).

Therapists

Therapist are licensed to assist in the examination and treatment of clients in in special ways (that is, as physical, occupational, or speech therapists). Their education varies, but usually involves four years degree program. DREAM-HOPE nurse collaborate with them and evaluate their work.

Social worker

Social worker are trained to counsel and refer clients to appropriate agencies. They have baccalaureate or master’s degree. DREAM-HOPE Nurses and health care provider work together with them to identify the best resources for the client, particularly when the client return home.

Case Manager

Case managers are responsible for managing client discharge form the hospital, or assisted Living Facilities and other residential facilities to home care. DREAM-HOPE nurse will work with case manager closely with discharge planning to home case, reviewing client case, identifying community resources, and meeting and achieving client service needs.

Chaplain

Chaplains offer spiritual support and guidance to clients and families. They work in agencies and/or provide services at the church of different, community, facility and at home. A client/family have a choice to request a Chaplin or practice their religious ritual with their time convenience
